All registered patient needs to pay $10 refundable registration fee send through zelle at [email protected]. If we don't receive $10 in 2 days after the registration, your registration will be automatically cancelled. Clinic personnel will contact you in 2-4 days after the submission to confirm your registration. Only age 18 and above are allowed for registration. (Clinic dates are every 2nd and 4th Saturday of the month from 1pm - 2:30pm)
