As a student in Biomedical Sciences on a MBA & Pre-Med Track, anda member of the IFN Clinic Student Society, I am initiating this surveybecause I am interested inbuilding a healthy Masjid community.
The information will be used to plan and implement evidence-based,preventative care health programs at IFN. The de-identified results may also besubmitted for publication to a peer reviewed journal in collaboration with our StudentAdvisor, Sr Nancy, inshallah.
Your participation is your consent to participate. There are no risks toparticipation. If you decide not to complete the survey, you may stop at anytime.
Your participation is voluntary, but you are strongly encouraged toparticipate since this survey will provide important information about thehealth needs at IFN, and regarding what programs would be of greatest interest and value to you and your family.
