IFN Clinic Student Society Cardiac and Diabetic Questionaire
Dear IFN Community Members,
Thank you for participating in this health screening survey at the Islamic Foundation North.
This survey, for adults age 21 and older, is designed to gather information about your cardiac and diabetic risk factors; two of the most prevalent medical conditions in our community.
This information will be used to guide the programming of our free clinic and the involvement of the IFN Clinic Student Society in developing Community Health projects.
Please answer the following questions to the best of your ability. Inshallah this survey should take approximately 5 minutes to complete.
Please do not put your name on this survey. Your responses will remain confidential and will be collected anonymously.
The anonymous data collected is crucial in shaping our healthcare initiatives. It may also be used in its de-identified form, for future IFN Clinic Student Society research projects.
